On November 11, 2017, after just 11 professional fights, he captured the vacant IBF light heavyweight title by defeating Enrico Kölling via a 12th-round knockout. His unification efforts continued, and by June 18, 2022, he had also secured the WBO title after defeating Joe Smith Jr. in a second-round technical knockout.

In a highly anticipated match on October 12, 2024, Beterbiev faced WBA (Super) champion Dmitry Bivol. The bout, held at the Kingdom Arena in Riyadh, Saudi Arabia, was a grueling contest that went the full distance. Beterbiev won by majority decision with scores of 114-114, 115-113, and 116-112, becoming the first undisputed light heavyweight champion in the four-belt era.
